The diabetes educators at South Pointe Hospital, a Cleveland Clinic hospital, are committed to the concept of quality patient education. Our goal is to assist you in gaining confidence as you embark on this lifelong journey of health care. Our comprehensive Diabetes Self-Management Program is available to inpatients, outpatients and the community. The outpatient program is recognized by the American Association of Diabetes Educators.
Who Can Benefit From Diabetes Education?
All persons with diabetes benefit from diabetes education as they learn self-management skills to control their diabetes and optimize their ability to live a productive and satisfying life.
The Diabetes Team
The team consists of your doctor, a registered nurse and registered dietitian, who are also Certified Diabetes Educators.
For Your Convenience
Your family and friends are invited to join you at any of these programs located at South Pointe Hospital in Warrensville Heights, the Community Health Resource Center at the Macedonia Recreation Center in Macedonia on Rt. 82, and South Pointe now offers Diabetes Education classes at the Solon Family Health Center at 29800 Bainbridge Road, Solon. So take advantage of one of these three convenient locations.
Diabetes Education Program
Registered dietitians, who are also certified diabetes educators are available for one-on-one teaching of meal planning, diet modification and weight control. The program offers four easy to follow classes spanning over a three-month period. Learn self-management skills to control diabetes and optimize a person's ability to live a productive and healthy life.
Classes focus on healthy eating, being active, monitoring, taking medication, problem solving, healthy coping and reducing risks.
The program includes an individualized consultation with a nurse and dietitian. Support groups are offered quarterly with guest speakers.
Insurance
Outpatient services may be reimbursable when your attendance is by physician referral. Your individual insurance policy defines the extent of your coverage. If you have any questions regarding insurance coverage, call the Diabetes Education Department at 216.491.7385.
Community Support
The Diabetes Management Program at South Pointe Hospital offers support services for your diabetes self-management needs. Our experts provide quarterly group meetings, free lectures and health talks. Non-Healing Wounds
About 15% of people with diabetes suffer from non-healing wounds. Diabetics are at a greater risk of developing chronic wounds because of poor circulation, slow healing times, vascular disease and neuropathy (numbness). Preventing infections and taking proper care of wounds, cuts, scrapes, and bruises are critical for those with diabetes.
