The staff of the Subacute Care Center provides intensive, short-term skilled nursing and rehabilitative care for our patients. This highly interdisciplinary environment provides the opportunity for patients to enhance their independence and self-reliance. The goal is to restore patients to their maximum level of functioning.

The Subacute Care Center is state licensed and federally certified for participation in the Medicare program.

Patients, along with family and friends can be assured that specially trained, dedicated professionals are providing the highest quality care around the clock.

In addition, they can feel confident that the full-service capabilities of the hospital's advanced technology is readily available should a patient require immediate medical attention or diagnostics.

Families are strongly encouraged to become involved in the patient's care and progress. Flexible visiting hours are designed to enable family and friends to support and assist the rehabilitation and recovery of their loved one.

Throughout their stay, patients will have the opportunity to continue under the care of their personal physician. The medical director, a physician specializing in geriatrics, provides medical direction and ensures that each patient receives the highest quality medical care. Our patients' physicians receive complete information regarding their patient's progress and follow-up upon discharge.

Each patient can benefit from our interdisciplinary care team which includes a physician, nurse, dietitian, social work and nurse case managers, recreation therapist and rehabilitative therapists.

Other speciality consultants are available as necessary. Among the services they provide are: comprehensive medical, mental status and social assessments; ongoing medical and psychiatric treatment; individual, family and group counseling; and linkage to needed resources and healthcare professionals.

Private rooms, televisions and telephones are available to patients. Also available are newspaper and mail delivery, library and religious services, and barber/ beauty shop services.

Patients are admitted to the Subacute Care Center under several financial programs, including Medicare or private insurance.

The Subacute Care Center is part of a seamless rehabilitative continuum of care, including acute rehabilitation, outpatient rehabilitation, and pain management.
