The diabetes educators at South Pointe Hospital are committed to the concept of quality patient education. Our goal is to assist you in gaining confidence as you embark on this lifelong journey of health care. Our comprehensive Diabetes Self-Management Program is available to inpatients, outpatients and the community.

 

Nationally Recognized 

The outpatient program is recognized by the American Diabetes Association (ADA) for meeting the requirements for the national standards for diabetes self-management education. 

 

In 1999, US News & World Report's 10th Annual Guide to "America's Best Hospitals" named South Pointe Hospital as one of the top 50 hospitals nationwide for the treatment of hormonal disorders such as diabetes. We received this honor by demonstrating the lowest mortality rate in the country. All of this recognition validates our commitment to excellence in patient care and our focus on customer services.

 

Who Can Benefit From Diabetes Education? 

All persons with diabetes can benefit from diabetes education as they learn self-management skills to control their diabetes and optimize their ability to live a productive and satisfying life.

 

The Diabetes Team 

Your health team consists of your doctor, certified diabetes educators (nurse & dietitian), and yourself. We believe that together this team can provide the ongoing support needed to maintain a healthy lifestyle.

 

The Program

Diabetes Education offers:

  • Inpatient diabetes education services are provided by certified diabetes educators, nurses, dietitians, and your one-on-one individual needs.
  • The program is a 3 month long comprehensive program and classes meet bi-weekly. Call 216.491.7385 to register.
  • Certified diabetes nurse educators are available for personal one-on-one teaching of all phases of diabetes management, including insulin instruction and blood glucose monitoring, as well as personal consultation, and problem solving.
  • Registered dietitians who are also certified diabetes educators are available for one-on-one teaching of meal planning, diet modification and weight control.

 

Insurance 

Outpatient services may be reimbursable when your attendance is by physician referral. Your individual insurance policy defines the extent of your coverage. If you have any questions regarding insurance coverage, call the Diabetes Education Department.

 

Community Support 

South Pointe also offers diabetes services through community outreach. Our experts provide support group meetings held quarterly and free lectures.
