PVD (Peripheral Vascular Disease) Risk Assessment



Please review the questions below:

  1. Do you have a family history of diabetes or cardiovascular problems?

  2. Do you currently have diabetes or cardiovascular problems?

  3. Do you experience impotence (men)?

  4. Do you smoke?

  5. Are you more than 25 lbs. overweight?

  6. Do you eat fried or fatty foods three times a week or more?

  7. Are you over the age of 50?

  8. Do you have any of these symptoms:

          •  aching, cramping or pain in the legs while you walk or exercise, that goes way after rest?
          •  tingling, numbness or coldness in your hands or feet?
          •  loss of hair on your feet or toes?
          •  irregular growth of fingernails or toenails?

 

If you answered YES to 3 or more of these questions, you are considered to be at risk for Peripheral Vascular Disease and should talk to your doctor.  If you do not have a doctor, call Medline at (440) 312-4533 for a free physician referral.
